Annual Permission and Medical Forms

Every year every Scout and adult leader must complete and turn in BSA's Consent and Medical Form, parts A, B, and C, which entails a doctor visit and attaching a copy of your health insurance card. If attending a National High Adventure trip, you must have your doctor complete the Part C specific for that base. All expire after one year. We strongly recommend parents make 10 copies of the completed forms - turn in 2 complete copies to the Troop, keep one into your car(s), put one in a ziplock inside each of your Scout's backpacks and duffel bags, and the rest handy to replace the former. Although you could upload a form, TT doesn't do that well. You can see when T68 thinks your record expires in the Medical Book. If the Troop (or Scout) doesn't have the form, you'll drive out to drop off the form or pick him/her up.
